彩色多普勒超声与磁共振成像在乳腺癌诊断中的应用价值

[摘要]目的 探彩色多普勒超声(CDUS)与磁共振成像(MRI)在乳腺癌诊断中的应用价值。方法 收集2014年6月~2016年5月我院收治的130例乳腺病变患者的临床资料。对照病理检查结果,比较CDUS弹性成像与3.0 T MRI对乳腺癌的诊断符合率以及肿块、毛刺征、钙化检出情况。结果 MRI对乳腺良性、恶性病变的诊断准确率分别为73.3%和67.4%,CDUS弹性成像对乳腺良性、恶性病变的诊断准确率分别为74.6%和75.6%,均具有一定漏诊率。CDUS弹性成像对肿块的敏感性较高,但对钙化、毛刺征的敏感性不高;MRI对毛刺征和肿块的敏感性较高,但对钙化的敏感性低,两者单独应用各有优劣。CDUS弹性成像结合MRI能进一步提高乳腺癌患者的肿块、毛刺征以及钙化的检出率(χ2=10.736、3.125、22.428,均P

[Abstract]Objective To explore the application value of color Doppler ultrasound (CDUS) and magnetic resonance imaging (MRI) in the diagnosis of breast cancer.Methods The clinical data of 130 patients with breast diseases admitted to our hospital from June 2014 to May 2016 were collected.Control pathological examination results,the diagnostic accuracy of breast cancer and mass,Mao Cizheng,calcification of CDUS elastography and 3.0 T MRI were compared.Results The diagnostic accuracy of MRI for benign and malignant breast lesions was 73.3% and 67.4%,respectively.The diagnostic accuracy of CDUS elastography in benign and malignant breast lesions was 74.6% and 75.6%,respectively,with a certain rate of missed diagnosis.CDUS elastic imaging was sensitive to the masses,but it was not sensitive to calcification and Mao Cizheng.MRI had high sensitivity to burr sign and mass,but had low sensitivity to calcification.There were own advantages and disadvantages in the two separate applications.CDUS elastography combined with MRI could further improve the detection rate of breast cancer patients,Mao Cizheng and calcification (χ2=10.736,3.125,22.428,P

本研究结果显示,CDUS弹性成像对乳腺良性、恶性病变的诊断准确率分别为74.6%和75.6%,而MRI对乳腺良性、恶性病变的诊断准确率分别为73.3%和67.4%,CDUS弹性成像与MRI均具有一定的误诊和漏诊率。乳腺癌的临床病理表现主要为钙化、毛刺征和肿块,目前临床也将这三种特征作为诊断乳腺癌的标准[14]。与MRI比较,CDUS弹性成像对肿块敏感性更高,且可反复检查,操作简便。此外,CDUS还能实时观察病灶内的血管走形,比彩超更能显示微小血管,进而提高了诊断率,但是CDUS对钙化、毛刺征敏感性不高。究其原因可能是CDUS弹性成像的优势在于判断乳腺肿块的大小,但对乳腺性质判断能力不足[15],因此具有一定的误诊和漏诊率。MRI对乳腺良性、恶性病变的诊断准确率分别为73.3%和67.4%,比CDUS弹性成像略低。MRI对毛刺征和肿块的敏感性较CDUS弹性成像高,但是在判断乳腺肿块的大小方面不如CDUS,这可能与MRI能对不同断层进行影像学分析有关,MRI借用对比剂以反映乳腺病灶血管数及通透性,在一定程度上弥补了CDUS的不足。MRI可从多个侧面对乳腺肿块进行观察,是乳腺癌术前评估及术后疗效判断的重要手段[16],但是MRI对钙化的敏感度也很低,也容易漏诊、误诊,且价格昂贵,从医学经济学考虑,CDUS要优于MRI。由于CDUS与MRI各有优势、劣势,为了能提高疾病的诊断准确率,本研究将CDUS弹性成像与MRI结合用于诊断乳腺癌,结果显示其能进一步提高乳腺癌患者肿块、毛刺征、钙化的诊出率(均P

综上所述,CDUS弹性成像和MRI单独用于诊断乳腺癌均具有较高的漏诊、误诊率,但将两者结合后,其对乳腺肿块、毛刺征、钙化的诊出率均明显提高,能为乳腺癌早期诊断提供参考,可供临床进一步深入研究。
