[摘要] 目的 探讨降钙素原区分细菌培养阳性患者主要致病菌的意义。 方法 选取包头医学院第二附属医院2015年12月~2016年12月住院患者252例,同时做细菌培养和PCT检测,分析革兰氏阳性菌、革兰氏阴性菌、真菌与PCT的相关性。 结果 120培养阳性患者,其中革兰氏阳性菌32(26.7%)例、革兰氏阴性菌69(57.5%)例、真菌19(15.8%)例。革兰氏阴性菌PCT中位数水平(1.31μg/L,IQR:0.231~9.255)高于革兰氏阳性菌(0.444μg/L,IQR:0.139~2.08),P

Significance of procalcitonin in division of main pathogens of bacterial culture

[Abstract] Objective To explore the significance of procalcitonin in division of main pathogens of bacterial culture. Methods 252 hospitalized patients in the Second Affiliated Hospital of Baotou Medical College from December 2015 to December 2016 were selected.At the same time,bacterial culture and PCT detection were carried on.Correlations between gram positive bacteria and gram negative bacteria,fungi and PCT were analyzed. Results Among the 120 positive patients,there were 32 cases of gram positive bacteria 26.7%,69 caes of gram negative bacteria 57.5% and 19 cases of fungi 15.8%.Median PCT level in gram negative bacteria was 1.31μg/L,IQR:0.231-9.255,which was significantly higher than that in gram positive bacteria with 0.444μg/L,IQR:0.139-2.08,P

近年PCT已经成为一种用于全身性细菌感染诊断与治疗监测的非创伤性临床实验室新指标,目前已广泛应用于临床[1-3]。本研究观察了120例细菌培养阳性患者PCT的变化,并比较不同病原体之间PCT的相关性,现报道如下。

1 资料与方法

1.1 一般资料

选取包头医学院第二附属医院2015年12月~2016年12月同时接受PCT检测以及细菌培养的住院患者252例,其中男150例,女102例,平均年龄(65.6±16.9)岁。分别记录患者细菌培养病原菌结果和PCT值。比较革兰氏阳性菌、革兰氏阴性菌、真菌和阴性组患者PCT 分布情况以及PCT阳性率情况。

1.2 纳入标准和排除标准

纳入标准:所选患者的PCT送检时间以及细菌培养送检时间相差不多于24h。所选患者经医院管理委员会批准,研究者知情同意。排除标准:若患者检测前已用抗生素,排除入组。

1.3 方法

1.3.1 细菌培养 血液、胆汁、腹水采用美国BD公司血培养仪9050检测,痰、尿液标本常规接种培养。细菌鉴定和药敏采用美国BD公司Poenix100全自动细菌鉴定仪。
