[摘要] 目的 评估应用锁骨钩钢板加喙肩韧带重建喙锁韧带联合中药治疗肩锁关节脱位的临床疗效。 方法 选择2006年10月～2010年10月我院收治的肩锁关节脱位住院患者80例的临床资料，将所有患者随机分为两组，A组40例采用锁骨钩钢板加喙肩韧带重建喙锁韧带联合中药治疗；B组40例采用锁骨钩钢板加锚钉重建喙锁韧带治疗。比较两组患者肩关节功能恢复时间及术前、术后JOA评分情况。 结果 两组治疗后的JOA评分，A组为（96.17±1.10）分，B组为（88.35±2.14）分，两组比较差异有统计学意义（t = 3.495，P ＜ 0.05）。80例患者全部进行了随访，时间为6～24个月，平均14个月，骨折均愈合良好。A组肩关节功能恢复时间为4～8个月，平均为（5.3±1.1）个月。B组为6～9个月，平均为（7.4±1.6）个月，两组比较差异有统计学意义（t = 2.982，P ＜ 0.05）。 结论 采用锁骨钩钢板加张力带或锚钉重建喙锁韧带联合中药治疗肩锁关节脱位手术操作简便，对肩部生理功能影响小，韧带重建可靠，内固定牢固，近期疗效满意。

Clavicular hook plate and coracoacromial ligament transferred to reconstruct coracoclavicular ligament combined with Chinese medicine in the treatment of acromioclavicular joint dislocation

[Abstract] Objective To assessment the effect of clavicular hook plate and coracoacromial ligament transferred to reconstruct coracoclavicular ligament combined with Chinese medicine in the treatment of acromioclavicular joint dislocation. Methods 80 patients with acromioclavicular joint dislocation in our hospital from October 2006 to October 2010 were randomly divided into two groups, 40 cases of patients in group A were treated with clavicular hook plate and coracoacromial ligament transferred to reconstruct coracoclavicular ligament combined with Chinese medicine, and 40 cases of patients in group B were treated with clavicular hook plate and anchor. Shoulder function recovery time and preoperative and postoperative JOA score of two groups were compared. Results The postoperative JOA score of group A and group B were (96.17±1.10), (88.35±2.14) respectively, the difference was significant (t = 3.495, P ＜ 0.05); 80 patients were followed up for 6 - 24 months; average 14 months, fracture healing was good. Shoulder function recovery time of group A was 4 to 8 months, an average of (5.3±1.1) months, that of group B was 6-9 months, an average of (7.4±1.6) months, difference was significant (t = 2.982, P ＜ 0.05). Conclusion Reconstruct coracoclavicular ligament with clavicular hook plate and tension band or anchor combined with Chinese medicine is easy and simple to handle, the influence on physiological function of shoulder is small, internal fixation and recent results were satisfactory.
