[摘要] 目的 分析牙本质折断恒牙的牙髓预后及其影响因素。方法 对行间接盖髓治疗且复诊时间大于2年的牙本质折断病例进行回顾分析,记录牙髓预后情况,回归分析影响牙髓预后的危险因素。结果 收集到牙本质折断病例205例,涉及患牙242颗。242颗患牙中,78颗(32.2%)发生了牙髓坏死,3颗(1.2%)发生了牙髓钙化。牙本质折断后行间接盖髓术的成功率为67.8%。复诊期间177颗年轻恒牙中141颗根尖孔闭合。冠折近髓程度与牙髓坏死率有相关性(P=0.001)。结论 牙本质折断恒牙行间接盖髓术的预后较好,冠折近髓程度是判断牙髓预后的重要指征。

学龄儿童为牙外伤的好发人群,发生率为6%~30%[1-5]。牙冠折断是牙外伤的常见类型,根据牙冠折断程度可以分为釉质折断、牙本质折断和冠折露髓;其中牙本质折断占25%~70%[4,6-9]。牙本质暴露后,细菌可能进入牙本质小管,从而导致牙髓炎症坏死。
牙本质折断预后的影响因素包括患儿年龄、就诊时间、牙冠折断的范围以及深度、牙周组织损伤情况等[10-12]。同时,学龄儿童恒切牙牙根处于发育阶
段,其根尖孔是否闭合对牙髓预后亦有影响[13]。本研究通过回顾242颗复诊2年以上的牙本质折断恒牙的牙髓预后情况,分析预后的影响因素。
1 材料和方法
1.1 临床资料
对1991—2008年就诊于北京大学口腔医院儿童口腔科的牙本质折断病例进行回顾分析。病例的纳入标准:1)牙本质折断(李宏毅分类法);2)治疗方式为氢氧化钙间接盖髓;3)复诊时间大于2年。排除标准:1)外伤前有牙体牙髓疾患或牙周炎症;2)外伤伴有挫入、脱臼、牙槽突骨折等严重牙周组织损伤。
1.2 牙髓预后的影响因素分度
将可能影响牙髓预后的因素进行分度(表1),影响因素包括外伤后就诊时间、初诊叩痛、冠折面积、冠折近髓程度、根尖孔是否闭合。
1.3 牙髓活力温度测试
记录牙髓活力温度测试冷测法的结果,分为正
常、敏感、迟钝、无反应四类。
1.4 牙髓预后情况
记录牙髓的预后情况。牙髓预后分为三类:1)牙髓正常:牙冠颜色自然,牙髓活力温度测正常,根尖片无病理改变;2)牙髓坏死:牙冠变色,牙髓活力温度测异常,叩诊不适,根尖区可见骨质密度减低影;3)牙髓钙化:牙冠变色,根尖片发现根管影像变窄、消失或模糊[13-14]。牙髓正常和牙髓钙化
视为治疗成功,否则为失败。
1.5 统计分析
采用SPSS 16.0软件进行统计分析,对影响因素作卡方检验后,应用Logistic回归得到危险系数,检验水准α=0.05。
2 结果
2.1 病例的基本情况
收集到205例符合纳入条件的牙本质折断病例,其中男性136例,女性69例,男女比例约为2∶1;年龄(9.2±1.7)岁。共涉及患牙242颗,其中上颌中切牙
231颗(95.5%),上颌侧切牙8颗(3.3%),下颌切牙3颗(1.2%);177颗根尖孔未闭合(年轻恒牙),65颗根尖孔闭合(成熟恒牙)。病例平均复查时间为(49.0±23.3)个月。
2.2 牙髓预后情况
在242颗患牙中,161颗(66.5%)牙髓正常,78颗(32.2%)发生了牙髓坏死,3颗(1.2%)在外伤后2.5、6、7年出现了根尖1/3牙髓钙化,但未发现根尖周病变。牙本质折断后行间接盖髓术的成功率为67.8%。
2.3 根尖孔是否闭合对牙髓预后的影响
根尖孔是否闭合对牙髓预后的影响见表2。年轻恒牙和成熟恒牙的牙髓坏死率分别为41.5%和28.8%,二者的牙髓坏死率无统计学差异(P>0.05)。复诊期
间177颗年轻恒牙中,141颗(79.7%)根尖孔闭合,其中46颗发生牙髓坏死;36颗(20.3%)根尖孔未闭合,其中5颗发生牙髓坏死。
2.4 牙髓预后的影响因素分析
年轻恒牙与成熟恒牙在各因素不同分度下的牙髓坏死率见表3。回归分析表明,冠折近髓程度与牙髓坏死率有相关性(P=0.001),是判断牙髓预后的重要指征。牙本质折断近髓的牙齿发生牙髓坏死的危险性是未近髓牙齿的4.48倍(OR=4.48)。
2.5 牙髓活力温度测试
242颗患牙中,186颗在复诊过程中完整记录了牙髓活力温度测试结果,其中:53颗温度测试正常者的牙髓坏死率为22.6%,12颗出现过温度测试迟钝者的牙髓坏死率为41.7%,121颗出现过温度测试敏感者的牙髓坏死率为34.7%,测试敏感持续时间平均为外伤后0.70年。
3 讨论
以往报道牙本质折断后牙髓的存活率为60%~97%[10,15-20]。各研究间牙髓存活率不同,与纳入病例是否为年轻恒牙、是否伴随牙周组织损伤、牙齿移位、病例随访时间等因素有关。本研究显示:牙本质折断后行间接盖髓治疗的成功率为67.8%。观察期间79.7%的年轻恒牙根尖孔闭合,观察期止仍有牙齿的牙根在发育中,根尖孔闭合的比例会更高。
本研究中,年轻恒牙牙髓坏死率为28.8%,成熟恒牙为41.5%。结果虽然没有统计学差异,但也提示年轻恒牙和成熟恒牙对于创伤的生物学反应可能有所不同。另外,有研究[10,13]认为,简单冠折伴
有严重牙周组织损伤时,牙髓坏死率会显著提高,尤其对于根尖孔闭合者。简单冠折伴有移位时,牙髓坏死率会由3%提高至66.6%[19]。本研究病例筛选时排除了伴有严重牙周组织损伤的病例,但保留了初诊叩痛这一指标。叩诊可以一定程度上反映根尖部及牙周膜的健康程度,本研究中初诊叩痛虽然与牙髓坏死无相关性,但其对成熟恒牙牙髓的预后有一定的提示意义,在临床上判断牙髓预后时对成熟恒牙和年轻恒牙应予以不同的考虑。
Olsburgh等[17]研究发现,当牙髓暴露并接触无菌异物时,并不会发生牙髓坏死,但外界细菌及其代谢产物进入暴露的牙本质小管并持续刺激牙髓组织时,就会导致炎症的发生,因此他认为细菌及其代谢物质为牙髓坏死的最主要因素。健康牙髓组织对外界刺激存在着防御机制:一方面,牙本质小管暴露后,牙髓牙本质复合体中小管液外流增加,牙髓组织中血液循环分泌纤维蛋白,可以在初期显著降低牙本质小管的通透性,减少外界物质对牙髓的刺激[21-24];另一方面,牙髓依靠修复性牙本质的形成抵
抗外源性刺激持续侵入髓腔[25],从而长期保护牙髓
组织。目前对于折断的牙本质常规采用对牙髓无刺激的材料覆盖断面以隔绝外界刺激,对于折断面近髓的患牙可用氢氧化钙制剂护髓后进行有效封闭。
本研究中,就诊时间对牙本质折断恒牙的牙髓预后无明显影响,与以往学者[13,26]的研究结果一致。但也有学者[27]认为,随外伤至初诊时间的延长,牙
本质折断患牙发生牙髓坏死的概率增加,超过3 d后呈现显著差异。
本研究中有3颗患牙在外伤后2.5、6、7年出现根尖1/3钙化,但未发生根尖周病变;牙髓钙化发生率为1.2%,与以往文献[17,28-29]报道的结果(0.2%~1.5%)相似。Jacobsen等[30]对122颗出现牙髓钙化的外伤牙
进行平均16年的观察,发现36%的患牙为牙髓部分钙化,64%的患牙为牙髓完全钙化,部分钙化者均未出现根尖周病变,而完全钙化者中有21%出现根尖周病变,,他认为创伤程度、牙根是否发育完全及牙髓钙化速度对判断根尖周病变的发生有提示意义。
恒牙萌出后牙根一般需2~3年才达到应有长度,而根尖完全闭合需3~5年;该过程中年轻恒牙对牙髓活力测试的反应阈值较成熟恒牙高[31-33],部分年
轻恒牙牙髓活力测甚至可能出现假阴性[32,34]。此外,外伤可造成牙髓不同程度损伤、根尖区神经纤维压力增大等,患牙往往需要一段时间才能恢复对温度刺激的正常反应[35-36]。外伤后连续的牙髓活力的检查,亦可为临床确定治疗方案提供重要依据;但需要综合临床症状、体征及根尖片情况,并纵向比较牙髓状态变化及牙根发育情况,谨慎进行牙髓治疗。
本研究表明:恒牙牙本质折断后行间接盖髓术的成功率为67.8%。冠折断面近髓是判断牙髓预后的重要指征。初诊叩痛对成熟恒牙牙本质折断的牙髓预后有提示意义。本研究未发现就诊时间及冠折面积对牙髓预后有显著影响。
