220例剖宫产患者围手术期抗菌药物应用情况对比分析

【摘要】 目的 分析抗菌药物整治前后剖宫产患者围手术期抗菌药物应用情况，为临床合理用药提供依据。 方法 采用回顾性分析方法，对我院产科2009年3~5月及2012年1~3月住院患者220份病历资料进行调查，探讨围手术期抗菌药物合理应用情况。 结果 抗菌药物整治前后剖宫产患者围手术期抗菌药物总的使用率分别为955%，944 %，平均用药时间分别为 47 d，38 d，最长预防用药时间分别为7 d，6 d。 结论 经过抗菌药物整治以后，剖宫产患者抗菌药物的使用率下降了11 %，平均用药时间缩短 09 d，二联及三联用药比例分别下降了229%，26%。最长预防用药时间缩短 1 d。但是仍存在抗菌药物的品种选择不恰当，联合用药不合理，抗菌药物使用时间过长，用药时机不规范，用法用量不规范，标本送检率低等问题。应进一步加强抗菌药物使用监管，促进围手术期合理用药。

【Abstract】 Objective To investigate antibiotic utilization during perioperative period in cesarean section cases, and to provide basis for rational medication in clinic Methods Retrospective study was done on 220 cases of cesarean section discharged from our hospital during March to May in 2009 and January to March in 2012 to discuss the rationality of perioperative antibiotic utilization Results The ratio of peroperative use of antibiotics for patients undergoing cesarean section, before and after control of antibacterials, were 955% and 944 % respectively, with the average durations of prophylaxis at 47 and 38 days and the longest durations at 7 and 6 days Conclusion After control of antibacterials, the rate of antibacterial utilization for cesarean section patients has decreased by 11 %, the average duration of medication has shortened by 09 day, the rate of twodrug and threedrug utilization has decline in 229% and 26% respectively The longest duration of perioperative antibiotic prophylaxis has shortened by 1 day However, there still exist some problems, such as inappropriate choice of drugs, irrational combination therapy, long course of treatment, improper medication timing, nonstandard usage and dosage, low rate of specimens tested etc It is necessary to strengthen supervision in order to promote rationality of perioperative antibiotic utilization.

作者单位：410008 湖南省妇幼保健院 外科围手术期合理使用抗菌药物是防治手术部位感染，减少医院感染的重要手段之一。按照《抗菌药物临床应用指导原则》［1］对围手术期预防用药的要求，对我院产科抗菌药物整治前后围手术期预防用药情况进行调研，旨在了解我院剖宫产围手术期抗菌药物使用的基本情况，为进一步规范抗菌药物合理使用提供依据。

1 资料与方法

11 资料来源 抽取我院2009年3~5月及2012年1~3月产科剖宫产手术病例220份，其中2009年3~5月112份，2012年1~3月108份，年龄21~40岁，平均25岁。

12 调查方法 采用回顾性调查的方法，按照《抗菌药物临床应用指导原则》、《围手术期预防应用抗菌药物指南》和卫生部办公厅《关于抗菌药物临床应用管理有关问题的通知》要求，调查患者围手术期抗菌药物的使用情况。
